Insurance Information

Please be advised that as of January 1, 2025 we are unfortunately no longer able to accept the following insurances:

• Any out of state Medicaid Plan

• CCBH that is not in Lackawanna, Luzerne, Susquehanna, Wyoming, Carbon, Monroe, Pike, or Wayne Counties

• Humana Gold Choice, PPO, or HMO

• United Healthcare Community Plan that is not from Pennsylvania

• Ambetter

• Carelon Behavioral Health

• Other insurances based on plan/coverage

If you have an out of state Medicaid Plan and currently reside in Pennsylvania you may come directly to the center without the need for an appointment and meet with a Liability Specialist who will help you apply for Medicaid in PA.
