Partial Hospitalization

Partial Hospitalization is an intensive day treatment program that is group based and designed to help consumers acquire skills to maintain functioning and prevent hospitalizations. It is available to consumers who are not making sufficient gains in the outpatient setting and are at risk of inpatient psychiatric hospitalization. Partial is also available to consumers who are discharging from a psychiatric hospital and need more support before returning to an outpatient setting. Partial offers group therapy, individual therapy, and psychiatric medication management. Partial groups run on-site Monday to Friday from 8:50 AM to 3:10 PM.

Bridge to Hope Partial Hospitalization is an outpatient program available to mental health consumers 18 years of age and older with a serious mental health disorder. The Partial program at SCC is an intensive clinical treatment program that focuses on managing, reducing, and resolving mental health symptoms as well as assisting program members in obtaining additional needed services in the community.

Referral Process: Individuals may be referred to Partial Hospitalization by their current mental health provider, who has conducted an evaluation and has determined they are in need of increased medication management and group therapy sessions. Individuals can also be referred to our Partial Hospitalization program by an inpatient facility as part of their discharge plan.

  • Admission Criteria for Bridge to Hope:

    • Must be 18 years of age or older and out of high school

    • Must have the capacity and desire to engage in group therapy sessions

    • Must not be a current danger to self or others

  • Therapeutic modalities:

    • Cognitive behavior therapy (CBT)— learning to change negative thought patterns and behaviors

    • Dialectical behavior therapy (DBT)—skills training in mindfulness, distress tolerance, emotion regulation, and interpersonal effectiveness

    • Acceptance and commitment therapy (ACT) – a person-centered treatment focused on your goals and values

  • Group Therapy Content (not an all-inclusive list):

    • Managing symptoms of depression, bipolar disorder and anxiety disorders

    • Developing skills to overcome social anxiety

    • Art—utilizing      personal      creativity      to express and process thoughts and feelings

    • Music—enhancing emotion regulation and relaxation techniques via song, rhythm, and lyrics

    • Movement—reducing stress and calming the mind through physical activities such as chair yoga, progressive muscle relaxation, and gentle stretching

    • Guided meditation

    • LGBTQ Groups
